At each step, our focus is on you. From learning about your goals and answering your questions to designing a care plan tailored for you and partnering you with the perfect caregiver, you are our priority. And it doesn’t stop there—we’ll be right there with you throughout the care process offering you and your family the assistance you need. After all, you’ve worked hard to get here. 1. Develop a Client Care Profile

When you call Jayach Care, we start by gathering information on how we can best help you. We’ll ask some questions to obtain a better grasp of your wishes. Then, we’ll plan an in-home visit to do a basic needs and environment assessment to establish the type of care you require. Whether its companion, personal, nursing or specialty care, we are there to answer any questions you may have and to provide the services you may need.

2. Create a Person-Centered Care Plan

Next, we’ll develop a person-centered care plan that is personalized to your requirements. The plan considers your and your loved one’s history, routines, and preferences in order to build support activities that will provide the most benefit. We will discuss the care plan with you in detail and amend it as you see suitable.

3. Identify the Best Caregiver or Health Care Professional for You

Allowing someone in your home is a major step. We are honored and happy that you put your trust in us. So, it’s crucial that we make the perfect caregiver or competent nursing match. We evaluate several criteria such as interest, personalities, and in-home medical assistance services needed. All caregivers and healthcare professionals are checked, including background and reference checks. They are also bonded/insured. Of course, your comfort counts the most. If you ever have any concerns, we will strive to locate you a new caregiver.

4. Deliver Extraordinary Care

We believe every engagement with Jayach Care should be extraordinary. Jayach Care’s caregivers live our vision and try to improve the quality of life for people they serve each day.
